SCHOLARSHIP APPLICATION

Use this application only if you meet the scholarship criteria.

To apply, please submit the following:
• A typewritten application (one original and four photocopies).
• A two-page typewritten essay describing your broadcasting goals as they relate to radio
  and the ways in which you hope to achieve these goals (one original and four photocopies).
• A resume (one original and four photocopies).
• Your official college transcript.
• Three letters of recommendation written by people other than relatives who can accurately
  evaluate your qualifications for this scholarship. The letters should include both your
  scholastic and personal strengths.

The letters of recommendation and the official transcript may be enclosed in the packet that is sent to the Bayliss Foundation as long as they are sealed. If these items are mailed separately to the Bayliss Foundation, please check with your contacts to ensure that the letters and transcript will be received by the scholarship application deadline, Thursday, April 30, 2009.

Applications will not be accepted electronically. Please do not send cassettes of sound checks.

By signing this application you agree that in the event you receive a Bayliss scholarship the money will only be used towards educational expenses. You also agree to let the Bayliss Foundation use your name in its literature.

Awards are made for the Fall semester, 2009.
The Bayliss Radio Scholarship recipients will be contacted by mail and/or email in late July.
Please make sure that your application contains a street or email address that will be accurate in July.

The Bayliss Broadcast Foundation is unable to pursue missing applications, transcripts or reference letters.
Incomplete applications will be disqualified and faxed materials will not be accepted.
Please submit applications after March 1, 2009.

PACKET MUST BE RECEIVED BY THE CLOSE OF BUSINESS ON THURSDAY, APRIL 30, 2009.
